Fisetin ve alfa lipoik asidin ağrılı diyabetik nöropati üzerine etkilerinin incelenmesi: fare modelinde in vivo davranışsal çalışma

Abstract
Bu çalışmada fare modelinde in vivo davranışsal çalışma ile fisetin ve alfa lipoik asidin ağrılı diyabetik nöropati üzerine etkilerinin incelenmesi amaçlanmıştır. Çalışmaya 48 adet BALB-C cinsi fare alınarak, herbir grupta 8 fare olacak şekilde 6 grup oluşturulmuştur. Grup 1: Kontrol grubu, Grup 2: Fisetin 3mg/kg, Grup 2: Fisetin 10 mg/kg, Grup 4: Alfa lipoik asid 50 mg/kg, Grup 5: Alfa lipoik asid 100 mg/kg ve Grup 6: Kombinasyon (Fisetin 5 mg/kg+Alfa lipoik asid 10 mg/kg) olarak belirlendi. Kontrol grubu hariç diğer 5 gruba yüksek doz streptozosin protokolü uygulanarak diyabet oluşumu sağlandı. Diyabetik nöropati oluşumu için deney süresi 8 hafta olarak belirlendi. Sekiz hafta sonunda plantar analjezimetre ile farelerin ağrı eşikleri değerlendirildi. Kontrol grubuna Fisetin çözeltisi olan DMSO, diyabet oluşturulan gruplara ise Fisetin ve Alfa lipoik asid belirlenen dozlarda intraperitoneal uygulanarak 10, 30, 60 ve 90. dakikalarda ağrı eşiği ölçümleri tekrarlandı.Fisetin 3 mg/kg ve Fisetin 10 mg/kg uygulanan gruplarda ağrı eşiklerinde uygulama sonrası istatistiksel anlamlı değişiklik izlenmedi (p=0,24, p=0,67). ALA 50 mg/kg ve ALA 100 mg/kg uygulanan gruplarda ağrı eşiklerinde 90 dakika sonunda istatistiksel olarak anlamlı kısalma saptandı (p= 0.05, p=0,001). Kombinasyon grubunda ise ağrı eşikleri bazale göre kısalmakla birlikte istatistiksel anlamlılık elde edilmedi (p=0,13).Sonuç olarak, Alfa lipoik asidin fare modelinde oluşturulan hipoaljeziyi düzeltmekte etkin olduğu ancak diyabetik nöropatide patogenezi hedefleyen ve tedavide etkili olabileceği düşünülen fisetinin hipoaljeziyi düzeltmediği bulunmuştur. Başka çalışmalarla fisetinin ve patogenezi hedefleyen farklı moleküllerin özellikle hipoaljezinin oluşmadığı erken evre diyabetik nöropatide etkinliğinin araştırılması uygun olacaktır. In this study we aimed to determine the effects of fisetin and alpha lipoic acid (ALA) on painful diabetic neuropathy in a experimental mice model in vivo.The study was consisted of 48 BALB-C type mice, and they were divided into 6 groups, each group consisted of 8 mice. Group 1: Control group, Group 2: Fisetin 3mg/kg, Group 3: Fisetin 10 mg/kg, Group 4: Alpha lipoic acid 50 mg/kg, Group 5: Alpha lipoic acid 100 mg/kg, Group 6: Combination group ( Fisetin 5 mg/kg and Alpha lipoic acid 10 mg/kg).Diabet was induced to all treatment groups except control group. The period for the occurrence of neuropathy was determined as 8 weeks. The pain thresholds of the mice were evaluated by plantar analgisemeter at the end of the study. After appliying intraperitoneally DMSO which is a solvent of fisetin to the control group and predetermined doses of fisetin and alpha lipoic acid to the diabetic groups, the pain threshold measurements were repeated on 10, 30, 60 and 90 minutes. Pain threshold results did not show a statistically significant in group 2 and 3 (p=0.24, p=0.67). However in group 4 and 5, pain threshold measurements were statistically significantly shortened at the end of 90 minutes. In the combination group although pain thereshold shortened when compared with baseline levels, it did not show a significant difference (p=0.13).As a result ALA was found to be effective whereas fisetin was found to be ineffective to ameliorate the hypoalgesia in this experimental diabetic model. Further investigations are needed to consider the efficiency of fisetin and other molecules which targets the pathogenesis especially in the early phases of the diabetic neuropathy in which hipoalgesia has not occured.
