Abstract
Giriş: Graves Hastalığı guatr, hipertiroidizm ve oftalmopati ile karakterize otoimmun bir hastalıktır. Çocuk ve ergenlerde hipertiroidizmin en sık nedenidir. İmmün sistem harekete geçerek TSH reseptör antikorları üretir ve bu antikorlar tiroid bezini uyararak dolaşımda tiroid hormon düzeylerinin artmasına neden olur.Klinik tanı tiroid hormonlarının ölçülmesi ile doğrulanır. Serum tiroksin(T4), triiodotironin(T3), serbest T4 ve serbest T3 düzeyleri yüksektir. TSH düzeyleri normal düzeylerin altına baskılanmıştır. Tiroid peroksidaz antikoru da dahil olmak üzere TSH reseptör antikorları pozitiftir.Graves hastalığının tedavisi antitiroid ilaçlar, tiroidektomi ve radyoaktif iyot verilmesi ile yapılmaktadır.Anti-tiroid ilaç tedavisine uzun dönem uyum güçtür ve bu durum başarı oranının düşmesinin temel nedenidir. Bu nedenle cerrahi olarak tiroid bezinin çıkarılması veya Radyoaktif İyot ile tiroid bezinin ablasyonu diğer tedavi seçenekleridir. Anti-tiroid ilaç kesildikten sonra relaps gözlenmesi, tedavi uyumsuzluğu, relaps varlığı ve tedavi yan etkisi durumunda diğer tedavi seçenekleri düşünülmelidir.Antitiroid ilaç tedavisi ile birinci veya ikinci yılın sonunda remisyon oranı %20-40 olarak bildirilmiştir. Küçük yaş (<5 yaş), büyük guatr volümü, düşük vücut kitle indeksi, yüksek sT4düzeyi ( sT4> 4 ng/dL, 50 pmol/L), başlangıçta ve tedavi kesilmeden önce yüksek TRAb düzeyi (>x4 kat), kısa tedavi süresi (<24 ay) remisyonu olumsuz etkileyen etmenlerdir. Bu yaklaşımdan yola çıkarak, kliniğimize başvuran Graves hastalığı tanısı alan hastaların başvuru semptomları ve bulguları, tiroid hormonları ve tiroid otoantikor düzeyleri, tedavi rejimleri, yan etkiler, tedavi sonuçlarını belirlemek amacı ile bu çalışmayı planladık. Method:Kliniğimizde 2004-2017 yılları arasında tanı alan Graves hastalığı tanılı hastalar retrospektif olarak değerlendirildi.Sonuçlar:Çalışmamızda Graves hastalığı tanısı alan 45 hasta değerlendirildi. Hastalarımızın %66,7'si kız, %33,3'ü erkektir. Hastalarımızın %35.6'sıprepubertal, %64.4'ü pubertaldir. Hastalarımızın %48,8'inde ailede tiroid hastalığı öyküsü mevcuttu. Hastalarımızın ortalama tanı yaşı 13,44 ± 2,78 yıldır.Başvuruda Graves hastalığı olan tüm hastalara antitiroid ilaç tedavisi başlandı.Olguların 10'u remisyon sağlanan, 3'ü remisyon sağlanamayan toplam 13 (%28,8) olguda medikal tedavi devam ettiği, sekizinde (%17,8) cerrahi tedavi, ikisinde (%4,4) RAI tedavi uygulandığı, 22'sinde (%48,9) medikal tedavinin tamamlandığı saptandı. Cerrahi ya da radyoaktif iyot tedavisinin en sık nedenleri relaps varlığı, medikal tedavi ile remisyonun sağlanamaması ve nodül varlığı idi. Antitiroid ilaç yan etkileri Graves hastalığı olan bir hastada gözlendi, alerjik döküntü gelişti. Tartışma:Graves Hastalığının prognoza etki eden faktörler ele alındığında; tanı yaşının relaps açısından önemli olduğu saptanmıştır. Anahtar Kelimeler: Graves hastalığı, hipertiroidi, radyoaktif iyot tedavisi Introduction: Graves' disease is an autoimmune disease characterized by goiter, hyperthyroidism and ophthalmopathy. It is the most common cause of hyperthyroidism at childhood and adolescence. The immune system activates and produces TSH receptor antibodies, which stimulate the thyroid gland and increase the thyroid hormone levels circulatory system. The clinical diagnosis is confirmed by measuring the thyroid hormones. The serum thyroxine (T4), triiodothyronine (T3), free T4 and free T3 levels are high. TSH levels were suppressed below normal levels. TSH receptor antibodies, including thyroid peroxidase antibody, are positive. The treatment of Graves disease is done with antithyroid medications, thyroidectomy and radioactive iodine medication. The long-term adaptation to anti-thyroid drug therapy is difficult and this is the main reason for the decrease in success rate. Therefore, the surgical removal of the thyroid gland or ablation of the radioactive iodine with the thyroid gland are the other treatment options. The observation of relapse after antithyroid drug is discontinued treatment adherence, other treatment options should be considered in case of relapse and side effects of presence.The remission rate at the end of the first or the second year with antithyroid drug therapy was reported as 20-40%. The small age (<5 age), large goitre volume, low body mass index, high Free T4 level (FreeT4>4ng/dL, 50 pmol/L), at the beginning and before the treatment discontinuation high TRAb level (<x4 times), short drug treatment (<24 months) are the factors that influences the remission negatively. Based on this approach, we have planned this study with the aim of determining the patients', who were admitted to our clinic with the diagnosis of Graves' disease, the admission symptoms and the findings, thyroid hormones and the thyroid autoantibody levels, treatment regimens, side effects and the treatment outcomes. Method: The patients diagnosed with Graves' disease in our clinic between the years 2004-2017 were retrospectively evaluated.Results: In our study, 45 patients with Graves' disease were evaluated. 66,7% of our patients are female and 33,3% are male. 35,6% of our patients are prepubertal and 64,4% are pubertal. 48,8% of our patients had a family history of thyroid disease.The average diagnosis age of the patients is 13,44 ± 2.78 years.All the patients with Graves' disease in the admission were started with antithyroid drug therapy. 10 cases were in remission provided, three of patient can not be remission a total of 13 (28.8%) continued medical treatment of patients, eight (17.8%) surgery, two (4.4%) RAI therapy is applied, in 22 (48.9%) medical treatment was completed.The most frequent causes of the surgical or the radioactive iodine treatment were relapse presence, being not able to provide remission with medical treatment and nodule presence. The antithyroid drug side effects has been observed in 1 patient with Graves disease, an allergic rash developed. One patient with Graves' disease was found to have thyroid papillary carcinoma. Discussion: When the factors affecting the prognosis of Graves disease are considered; the age of diagnosis was found to be important for relapse.Keywords: Graves' disease, hyperthyroidism, radioactive iodine treatment
