D vitamini eksikliği olan prediyabetik ve diyabetik hastalarda D vitamini replasmanının ileri glikozillenme parametreleri üzerine etkisi

Abstract
Amaç: D vitamini eksikliğinin Tip 2 diyabetes mellitus(T2DM) gelişiminde bir riskfaktörü olduğu ve vitamin D'nin insülin sekresyonu ve insülin direnci üzerinde rolüolduğuna dair güçlü veriler vardır. Bu çalışmada; D vitamin eksikliği olan prediyabetikve tip 2 diyabetik hastalarda d vitamini replasmanının ileri glikasyon ürünleri üzerineetkisini incelemek amaçlanmıştırYöntem: Çalışmaya vitamin D düzeyi <20 ng/ml olan ; 40 tip 2 DM yaşları 52±7,7 ;K/E: 28/12, 40 prediyabetik yaşları 47,3±8,9; K/E:34/6 ve 40 sağlıklı kontrol yaşları38,5±9; K/E:37/3 dahil edildi. Her üç gruba oral D vitamini 50.000 ü/Hafta 8 haftaboyunca yüklendi ve 1500 ü/Gün idame tedavisi verildi. Tedavi öncesi ve 12. Haftasonunda, vücut kitle indeksi, sistolik ve diyastolik kan basınçları, cilt ileri glikasyon sonürün birikimi (AGE) ,serum karboksimetillizin (CML), açlık kan şekeri, HBA1C,25(OH)D vitamin, kalsiyum, fosfor, parathormon ve kas gücü değerlendirildi. İstatistikanalizinde Mann Whitney U, T-TEST, Kruskal-Wallis test (nonparametrik ANOVA ),korelasyon analizi ve multiple regresyon analizi kullanıldı.Sonuçlar: Her üç çalışma grubunda tedavi sonrası serum 25(OH)D düzeylerinin anlamlıolarak yükseldiği izlendi(p<0,0001). Her üç çalışma grubunda tedavi sonrası HbA1C veaçlık kan şekeri düzeylerinde anlamlı değişiklik izlenmedi(p>0,05). Karboksimetillizindüzeylerinde her üç grupta anlamlı artış oldu(p<0,05). Cilt AGE birikimi diyabetikgrupta sağlıklı kontrol grubuna göre anlamlı olarak yüksek izlendi(p<0,05). Ancak Dvitamini replasmanı sonrası her üç grupta da cilt AGE düzeylerinde anlamlı azalmaizlenmedi(p>0,05). Cilt AGE birikimi ile HbA1C düzeyi arasında pozitif korelasyonizlendi(r =0.32,p<0.001). Serum 25(OH)D düzeyleri ile HbA1C ve cilt AGE düzeyleriarasında korelasyon izlenmedi(p>0,05) ve serum karboksimetillizin düzeyleri arasındapozitif korelasyon izlendi(r=0,28, p<0,0001). Tedavi sonrası kas gücünde her üç gruptaanlamlı artış saptandı (p<0,0001)Yorum : D vitamin replasmanı serum D vitamin düzeylerini arttırmıştır. Ancak 3aylık yükleme tedavi glisemik parametreler üzerinde anlamlı bir düşüşe yol açmamıştır.Serum karboksimetillizin düzeyleri tedavi sonnrası anlamlı olarak artmıştırBu sonuç literatürdeki ilk bulgudur. Cilt AGE birikiminin daha uzun dönemli olduğugöz önüne alındığında uzun dönemli takipler ve serum AGE ölçümleri ile D vitamini veileri glikozilasyon ürünleri metabolizması arasındaki bağlantı anlaşılabilecektir. Aim: There is strong evidence regarding vitamin D being a risk factor for Type 2Diabetes Mellitus (T2DM) and that vitamin D has a role on insulin secretion andresistance. The risk for developing diabetes was found to be higher in those with vitaminD deficiency compared to the healthy people. In the current study, we aimed toinvestigate the effects of vitamin D on advanced glycation end-products in prediabeticand type 2 diabetic patients with vitamin D deficiency.Methods: 40 type 2 diabetic patients with a mean age of 52±7,7 years (F/M: 28/12), 40prediabetic patients with a mean age of 47,3±8,9 (F/M: 34/6), and 40 healthy controlswith a mean age of 38,5±9 (F/M: 37/3) with the levels of vitamin D <20 ng/ml wereincluded in the current study. All three groups were given oral vitamin D 50.000 U/w asloading dose for 8 weeks and it was tapered to maintenance dose to 1500 U/d. All threegroups were evaluated with pretreatment and 12th week measurements of height,weight, waist circumference, systolic/diastolic blood pressures, advanced glycation endproducts(AGE) in skin, serum CML, fasting blood glucose, HbA1c, 25(OH) vitamin D,calcium, phosphorus, parathormone and their muscle strengths were assessed. MannWhitney U, T/Test, Kruskal-Wallis test (nonparametric ANOVA), correlation analysesand multiple regression analyses were used for statistical analyses.Results: All three groups showed statistically significant increase in their serum 25(OH)vitamin D levels (p<0,0001). There was no significant difference in HbA1c and fastingblood glucose levels in any of the groups after the treatment (p>0,05). Serun CML levelsincreased significantly in all three groups (p<0,05). Skin AGE deposition was observedsignificantly higher in diabetic group compared to healthy control group (p<0,05);however there was no significant decrease in skin AGE levels in any of the groups aftervitamin D replacement (p>0,05). Skin AGE deposition and HbA1c levels werepositively correlated (r =0.32,p<0.001). Serum 25(OH)D levels did not show anycorrelation with those of HbA1C and skin AGE (p>0,05) and showed positive correlationvto serum CML levels (r=0,28, p<0,0001). All three groups had statistically significantincrease in their muscle strength after treatment (p<0,0001).Discussion: Vitamin D replacement increased serum vitamin D levels in all threegroups; however it did not result in significant decrease in glycemic parameters after a3-month treatment. Serum CML levels significantly increased after treatment and thisincrease was thought to be due to weight gain. This finding is the first in literature.When AGE deposition in skin is considered a long-term effect, longer periods of followupand serum AGE measurements may confirm the correlation.
